We provide a complete range of care that does not stop at inpatient, but follows to the skilled nursing facility and to the patient’s home. Our hospitalists also work closely with case managers and social workers to ensure all needs are met, not only for the patients, but also their families.

PHP achieves its success through the use of sophisticated technology, a centralized database, communications with primary care physicians, specialists, case managers, social workers, patients and families to facilitate a comprehensive approach which includes timely patient care, frequent follow up, patient education and compliance.

Hospitalists

Our physicians have been chosen due to their achievements and medical skills. Expertise, teamwork, and commitment to quality compliment the Physicians Hospitalist Partners' mission.

All physicians in this group are board certified and hold privileges at all hospitals in the Central Florida area. Physicians are full time employees of PHP. They practice exclusively in hospitals or acute, sub acute and long-term care settings. Typically, a team of physicians will cover a defined local market based on proximity, volumes, and travel time between facilities.

Transitional Care

PHP registered nurses ensure care is coordinated during the patients' stay in the hospital and skilled nursing facility, optimizing the use of health care services and arranging for discharge as soon as is medically appropriate. This applies to an acute care hospital, a sub-acute facility, or an inpatient specialty treatment program.

  • Working under direct supervision of a PHP physician in delivering care to the patient
  • Accompanies physicians in making patient care rounds
  • Coordinates patient care and initiates patients
  • PHP’s case managers are also available to participate in team conferences, assist with education, advanced directives and community information
