PHP hospitalists are full-time, acute primary care specialists focusing on inpatient care.

We have created a unique link with primary care physicians and patients. This link empowers physicians to effectively manage both their inpatient and outpatient practices, thus increasing economic efficiency while enhancing quality of care.

HISTORY

Our founder, Dr. Nayana Vyas, sought to create a system of care that would improve the traditional model of hospital care. In 2001, she established Physicians Hospitalist Partners, providing a full range of services; dedicating itself to the most cost effective use of resources leading to enhanced quality care, as well as reduced length of stays in acute care settings.

OUR MISSION

PHP delivers acute care management of adult and geriatric population with a focus on coordination and integration of care with patients' primary care physicians and specialists. PHP team of experts emphasize on transition of care from the hospital to an outpatient setting in an effort to provide a seamless process of transition for patients and their families.
